Summary
Despite his previous resolve to get better about not being so clingy and needy after his realization, Jim actually felt like he was getting worse.
Bones’ quarters were practically his now and he slept there most nights.
When Bones visited Jim’s for a drink, he always sat as close to the doctor as possible without actually sitting on his lap (if they were using the couch, anyway).
He tried to keep him on the bridge as long as he could, even when Bones probably had better things to do.
And the man never blinked an eye, never gave him a weird look, and never said anything.
It frustrated Jim to no end and he fully intended to find out exactly why Bones was just letting him.
Or: Jim discovers he’s obsessed with, and may or may not have a slight (raging) co-dependency on, his best friend. Bones discovers Jim is only just discovering this (and doesn’t realize how Bones feels about him). Shenanigans ensue.

“Southern belle,” the man whispered fondly into his ear. McCoy could only nod in return.
He felt so warm. So right.
Eventually, the party had to end and the illusion shatter.
Or: Jim and Bones meet at an Academy masquerade ball instead of the shuttle. Jim finds out McCoy’s real identity within days, but Bones doesn’t realize who it is he danced with that night… shenanigans ensue.

McCoy returned his gaze to the ceiling, certain Jim was about to ask his next question in a similar fashion to how people used to drop atomic bombs on each other centuries earlier.
“Why’d you do it?” Jim’s voice dropped lower, something openly curious in his tone, but a note of certainty, surety, below that.
Which was what prompted McCoy’s own answer.
“You know damn well why.”
Or: After saving Jim’s life (i.e. resurrecting him), Bones waits for Jim to wake up again, and when he does, is confronted with the reality that he’s in love with his best friend.

Summary
Pathetic as it might have been, McCoy wished he was the person Kirk could confide in, feel safe with, be himself around. He found himself fantasizing about it. How they'd meet, how Kirk would quickly realize he could trust McCoy, how they'd spend time together away from everyone else just to be themselves.
But McCoy was a realist, a doctor, and divorced. Relationships didn't work like that, he and Kirk weren't even friends, and things were rarely ever so easy in life.
He allowed himself his little fantasy, anyway. He wasn't harming anybody with it, after all, and Kirk never need know. They'd both graduate the Academy and go their separate ways and McCoy would probably never see the kid again.
Or: McCoy has a crush on one James T. Kirk, but thinks Jim has no idea who he is. Turns out, Jim has been pining just as much as Bones.

“Jim, just fucking kiss me already,” Bones breathed out, his expression almost in awe, hopeful and disbelieving all at once. It was the exact look Jim would’ve expected him to have in reaction to finding out there was a cure for his disease, not to Jim’s rant.
“What?” Jim said dumbly.
“Kiss me.”
Or: Bones has xenopolycythemia, Jim is not happy about it, something something he gets cured and they get together :)

And then it hit him. Jim never did anything the normal way. He didn’t feel safe doing anything the normal way, which was why he seemed reckless to some people, downright idiotic to others, and the exact reason he was one of the few omega captains in Starfleet (and the youngest on top of that).
If Jim wanted something, wanted a bond (with his best friend, no less), he wasn’t just going to bring it up as a casual conversation topic during a drinking session.
Instead, Jim would put himself in a ridiculous situation just to test whether McCoy even wanted him or not.
Or: Jim gets himself bonded to Bones and Bones feels guilty about it until he realizes Jim actually wants him. Cue the love confessions :)

Besides, Jim was everything to him now. Truly. McCoy was head over heels for the kid.
In fact, he was so far gone, Jim was basically all he could think about these days, no matter how hard he tried not to.
It almost felt like he was on some slow descent to insanity, being completely consumed by something he’d never felt for anyone before and had only seen in holos or heard in dumb pop music and yet, deep down, hoped to experience his whole life.
Or: Bones falls madly in love with Jim and tries to pretend he totally didn’t even though he totally did
