It's true, he's losing a bit of sleep. And yeah, he's having a hard time relaxing. And sure, anytime Luigi isn't near him, his heart races, and his brain tries to convince him that his brother is hurt, or lost, or dying.
But it's fine. As long as Luigi is always by his side, then everything is fine.
Mario is fine.
Or, after the events of the movie, Mario develops a new fear he doesn’t know how to deal with. Separation anxiety, am I right?

And he still got sick.

A strange affliction is causing Luigi to sleep more and more each day. Worried about his constant collapses, Mario enlists Princess Peach’s help in looking for a cure, leaving Bowser to play nurse and watch over the dreaming brother.
To Bowser, this is just an easy favour to make Mario owe him in the future. However, he soon starts to find himself getting much too invested in the wellbeing of a man who may soon never wake up again.

At first, Mario's face looked perplexed. Eyebrows furrowed together, his mouth parted as if he never would expect such a fine piece to be made from the likes of his scaly claws. Then, at Luigi’s piqued interest, he too took on a surprised look on his face. His eyes subtly looked his brother up and down, as if he was rethinking what Luigi thought of Bowser.
They turned to look at each other. Bowser raised his painting, expecting the same complement as before. It would have been like an apology. Maybe, now that he was a different koopa, they didn’t need to be enemies. An olive branch. Forgiving him as he did the human.
“It’s trash.”

Luigi had it all.
He was taller, thinner, younger, richer, owned a mansion and a cute dog, he even jumped higher...
And he was more popular than Mario.
People knew of Mario, sure. Peach and her staff particularly held him in quite high regard.
But in terms of the general public? They LOVED Luigi. Many didn't even know who Mario was.
"OH, YOU'RE the Mario in "Super Mario Bros"? I thought it was just about Luigi's last name!" was something Mario got a fair amount.
Some people just saw Mario as the older, fat brother of the great Luigi. He was completely overshadowed by his little bro.
It'd be enough to get under anyone's skin, really.
...Anyone except Mario.
