Summary
"Do we have any painkillers?" Sam asks as Dean snaps an ice pack, taking it from him to apply to her eye.
"Yeah, in my bag. Come on." Dean stands, putting a hand to Sam's shoulder. He guides her into the bedroom, lets her sit on her bed as he gets her some pills and some water.
The silence returns as she takes the pills, finishing the water with another sip, the glass tinking as it's placed on the side table. Just as Dean is slipping back into his thoughts—back into guilt and remorse, Sam speaks again.
"What happens now?"

Set sometime after Season 4 Episode Yellow Fever, where the threat of Lucifer is high. Inspired heavily by a book about possession I recently read.

“Dean?” he asked, voice small.
“Hm?” Dean hummed, still not looking at him.
“Can you pull over?”
“We’re on the highway, man, no I can’t pull over. Why?” He finally turned his head, an annoyed expression on his face that melted away the second he saw Sam, pale and shaky, raising one hand to cover his mouth and swallowing thickly again.

A week later, Sam woke up feeling like he’d been run over by a train. His body ached, his head was pounding, and his nose was stuffy. He pushed himself up on one elbow and coughed into the other one.
“Jeez, Sam. Need a drink?” Dean asked from where he was sitting on his own bed. He frowned when he looked over at Sam. “You okay?”
Sam nodded, rubbing at his eyes. “Think I’m sick.”

Sam took a deep breath. “I dunno. Feel kinda weird.”
“Weird how? Bad weird?” Dean glanced over at his brother, glad they were on an empty stretch of highway.
“Like maybe I’m coming down with something?” Sam shrugged. “It’s probably nothing.”

“Why’re you here?” he asked. Dean stopped short, frowning and lowering the pills in a mix of hurt and confusion.
“Because I’m your brother,” Dean said firmly. “Why wouldn’t I be here?”
Sam frowned at that, looking almost confused. Dean’s heart pounded with worry.
“But… you were with the angels. I… I left,” Sam said, voice almost hesitant. Dean sighed, understanding washing through him.
“We’re at Bobby’s house, Sammy,” he said, motioning to the familiar room. “We’re together. It’s okay. Now will you please take this medicine-”
But Sam was already moving, pushing himself away from Dean and shaking his head hard enough that he swayed, almost losing his balance. Dean reached out frantically to steady him, but Sam whimpered at his touch and pulled away like he’d been burned.

Or, Sam gets sick for the first time since the apocalypse - since his and Dean's fight - and in his fevered state, some hidden thoughts and worries of his slip out. Dean makes sure to shut down his doubt quick.

Bobby asked Dean and Sam to take care of a vampire nest. It did not go smoothly. With Dean hurt, Sam hauls his brother to the nearest motel to patch him up and is determined to take care of him… even overlooking his own health in the process. And when neither brother is capable of taking care of the other, of course it’s Bobby to the rescue. Shameless hurt/comfort and good old fashioned sickfic.

Sam rubs his temples with a sigh. He leans over his textbook, wondering how long he can hold out on studying despite how hard his head is pounding. Black spots dance across his vision, but he turns back to the open page. He keeps reading for at least fifteen more minutes before black spots start dancing across his vision.
“Come on, not now,” he mutters. “I just want to finish this chapter. Well, this is what I get. It’s all the head injuries as a kid that are coming back to bite me in the ass in the form of developing migraines.”

Summary
Sam's always thought it's this fiction-thing, biting your lip until it bleeds. It seems dramatic and all, but short of tearing off too much dry skin or having really dry lips to start with, it takes more pressure than most people are willing to exert, really, to break the skin of the lip and make it actually bleed.
The pain of broken skin is absolutely nothing, though, to the feeling of Lucifer's hands rooting around in his innards (with that sort of gleeful detachment that the fallen archangel must have spent millennia cultivating) and tearing out whatever displeases him.
In other words... Sam wakes up from a Cage trauma-fueled nightmare and can't shake the dream. Dean helps by turning on the heater in high summer and sweating through it. In the end, there's nothing like a bit of blasphemy to shake the cold in his bones.

Summary
Dean shoves the gun into his waistband and runs out when he hears the scream.
He's expecting looming Darkness and wild-eyed zombies. He's expecting mass-death and cars crashed and a cityscape on fire.
Somehow, what he sees is a lot worse: the sky is clear, everyone is perfectly human, and... Sam is on the pavement, on his side, limbs jerking and spine arching.
In other words... Dean avoids Sam's every attempt to talk about things after the Darkness is released, finally boiling over and lashing out at Sam. Sam, though, has more to worry about than the darkness.
